Abstract

"To improve the health and quality of life of Missourians by decreasing the occurrence of emergency department visits and in-patient hospitalizations related to asthma exacerbations. We have chosen to evaluate models that focus on improving patient education, providing practical training and inclusion of environmental assessments to improve outcomes. Much of our target group belongs to low socioeconomic and minority populations that often have difficulty with transportation and continuity of care. They are at high risk for frequent ED visits and hospitalization. Demographic and ED visit data for the state and county was reviewed. Asthma was identified as a primary concern. A literature review was conducted to determine feasible interventions to improve accessibility and quality of care through asthma management programs (AMP)."--Objective and methods.
