Abstract

Background Delirium is a complex condition characterized by acute changes in mental status, attention, and behavior. For patients who undergo anesthesia that requires ICU stays, nearly 60% of these patients experience delirium. Delirium is independently linked to increased postoperative complications, higher mortality rates, and increased length of stay. The Cardiac ICU receives many post-anesthesia patients and was seeing an increase in delirium prevalence and resource allocation. The QI objectives are to increase delirium screening using the CAM-ICU, have documentation of delirium management for patients CAM-ICU positive, and high compliance with nursing staff completing the educational intervention. Methods Evidence-based resources were collected on CAM-ICU, ABCDEF bundle, and sleep hygiene practices to assemble a pre-recorded educational video for nursing staff. Knowledge assessments were completed with nursing staff before and after the educational video. QR codes were utilized to track the usage of nurses using the ABCDEF bundle and sleep hygiene prevention measures. Results Wilcoxon Signed Rank test revealed 8 of 10 questions on the knowledge assessment to be statistically significant, proving nursing knowledge improvement. CAM-ICU screening increased 33.96% from time frame one to time frame two. Of the patients CAM-ICU positive, delirium management measures were documented 37.9% of the time. Conclusions Continue delirium management practices in the Cardiac ICU and work towards having delirium prevention and management documentation added to the EHR to accurately track utilization.
