"Please include sample identification on each sample. Take a representative sample by sub sampling randomly. Mix the sub samples and submit about one pound of sample for analysis. When submitting samples directly to the lab, send a duly filled sample information form along with the check for amount due as payment. When submitting through the counties or firms that has account with us, we will be sending an invoice for the payment due by end of the month. To pay by check: Make checks payable to MU Soil Testing for the total amount due. The check must be sent along with the sample unless you have an account set up with the lab. The lab also accepts purchase orders; contact the lab to discuss this process. Samples will be discarded after 30 days." -- first page
