Abstract

This capstone project aims to address the challenges faced by older adults, aged 65 and above, in maintaining independence and engagement in daily activities for successful aging. The project was conducted in collaboration with Navigate Wellness, located in Columbia, Missouri, which assists older adults in making informed healthcare decisions to enhance their quality of life. The primary objective of the project was to develop and implement a quality improvement measure to educate Navigate Wellness staff and clients on factors influencing successful aging. A questionnaire was created to assess clients' perceived levels of independence in physical activity, productive activities, and nutrition management. The questionnaire was informed by existing literature and aimed to empower older adults to engage in activities that promote their independence and reduce hospital admission rates. The study highlights the importance of educating older adults, caregivers and healthcare professionals on age-appropriate interventions to support successful aging. The findings suggest that enhancing client engagement in meaningful activities can positively impact their physical and mental well-being, thus prolonging their ability to live independently at home. The project promotes the need for continued efforts to bridge the gap between client independence and the ability to age in place successfully.
