Abstract

Purpose: This study describes how adult transgender people hope and envision their experience of transgender affirmative nursing care. Problem: The well-being of transgender people seeking health-related care is impacted by their interactions with registered nurses, who have shown a lack of knowledge, negative attitudes, and poor behaviors. Significance: Transgender affirmative nursing care is essential for enhanced healthcare experiences. Nursing education often lacks content addressing the unique needs of transgender adults. To address this gap, it is imperative to incorporate elements of transgender affirmative nursing care into nursing curricula. Innovation: Previous research has not adequately captured the specific care preferences of transgender individuals. This study describes transgender affirmative nursing care as hoped and envisioned by adult transgender persons. Design: An inductive qualitative research design, guided by naturalistic inquiry, was used. Data were collected using a study-specific interview guide, allowing for an organized summary of transgender affirmative nursing care as envisioned by participants. Setting: Participants were recruited through the Washington University Transgender Center. Sample: Purposeful and snowball sampling methods were used to recruit participants who could provide insights into their experiences. Eligibility included the ability to speak and understand English and self-identification as transgender. Procedures: After institutional review board approval, participants were consented and scheduled for individual semi-structured interviews conducted by the primary investigator. Instruments: The primary investigator, qualified with graduate-level training in qualitative methods, utilized a study specific semi-structured interview guide to elicit detailed responses from the participants. Analysis: An inductive content analysis approach was utilized during transcription verification and coding to establish categories reflecting individuals’ experiences and visions when receiving transgender affirming nursing care. Findings: Participants highlighted how healthcare settings and body language can influence fears of mistreatment. Intentional misgendering and failure to acknowledge such errors lead to feelings of disrespect, causing many to forgo necessary medical care. The findings underscore the importance of implementing comprehensive training and policies within healthcare facilities to create inclusive environments and enhance nursing education, aligning with Rivera’s Gender Affirming Nursing Care (GANC) theory to better support transgender individuals.
