Abstract: Where diagnosis is essential and conventional testing proves equivocal, amniocentesis with injection of indigo carmine dye is the most definitive test. The most widely available of the newer biochemical assays, the plasma alpha-microglobulin-1 assay, appears to offer improved accuracy compared with conventional methods, but the clinical significance of a positive test, particularly in the setting of labor, is unclear.
