What is the differential diagnosis for an elevated monocyte count in a patient with an acute febrile illness?

Monocytosis can have many causes in a febrile patient with an acute illness. Etiologies can be broadly grouped into hematologic disorders, infections, inflammatory causes, and miscellaneous causes.
