The initial workup for an infant with persistent hypoglycemia is a through history and physical exam, a determination of fasting glucose, and an assessment for sepsis. Further laboratory evaluation may include serum insulin, growth hormone, free fatty acids, lactate, uric acid, ammonia, cortisol, thyroid-stimulating hormone (TSH), and insulin-like growth factor-binding protein-1 (IGFB-1).
