Is prophylactic anticoagulation safe and effective in older adults with atrial fibrillation?

Older adults with atrial fibrillation have fewer strokes with oral anticoagulation (OAC) compared with no treatment or antiplatelet therapy, but do have a small increased risk of intracranial hemorrhage (SOR: A, meta-analysis of RCTs). Dabigatran 150 mg PO BID may be more effective than warfarin for preventing strokes with a similar risk of bleeding events (SOR: B, single RCT).
