Is expectant management safe and effective for first-trimester miscarriage? Evidence-Based Answer: Expectant management for at least 2 weeks is a safe and acceptable alternative to medical management of first-trimester pregnancy loss (SOR: A, meta-analyses of RCTs and an observational study). Waiting 6 to 8 weeks may increase successful miscarriage without risk of infection, but may increase the risk of bleeding and need for surgical intervention (SOR: A, meta-analysis of RCTs). Studies beyond 8 weeks are lacking, and therefore a firm endpoint of safety has not been demonstrated.
