Should patients on chronic PPI therapy be placed on calcium supplementation to prevent fractures? Evidence-Based Answer: The answer is unclear; the use of proton pump inhibitors (PPI) is associated with an increased risk of hip and vertebral fractures (SOR: B, meta-analysis of cohort and case-control studies) but does not appear to alter the incidence of osteoporosis in 5 years of use (SOR: C, case-control study using disease-oriented outcome). It is prudent to advise patients on appropriate PPI therapy to take the recommended daily intake of calcium (SOR: C, expert opinion).
