Is inhaled insulin a safe and effective alternative for patients who need insulin? Evidence-Based Answer: Inhaled insulin is as effective as subcutaneous (SQ) insulin in lowering HbA1c in patients without pulmonary disease (SOR: C, disease-oriented evidence). Inhaled insulin is not associated with changes in pulmonary function (SOR: A, consistent RCTs), but does cause cough in about 8% of users. It may also produce 20% more hypoglycemic episodes than SQ insulin (SOR: B, single RCT).
