Does strict control of glucose levels in pregnant women with diabetes prevent birth defects in offspring? Evidence-Based Answer: Maintaining fasting and postprandial glucose levels at or near normal levels has not been consistently shown to decrease the rate of congenital malformations if initiated after conception. For women with diabetes who desire to become pregnant, blood sugar control optimized during both preconception and gestation has been associated with a decrease in the rate of congenital malformations (SOR: B, systematic review, RCT, and cohort studies).
