What is the best management of patients who experience a transient ischemic attack (TIA) while taking aspirin?

Aspirin plus extended-release dipyridamole is superior to aspirin monotherapy in preventing recurrent strokes in patients with a TIA or minor stroke, whereas no difference was noted between aspirin plus dipyridamole and clopidogrel in this risk group. No difference was apparent in stroke rates between aspirin plus clopidogrel and aspirin alone in patients with cardiovascular disease or risk factors alone (SOR B, consistent high-quality RCTs).
