What agents should older patients with atrial fibrillation take to prevent stroke?

Elderly patients with atrial fibrillation should be given anticoagulation agents to preven stroke. Adjusted-dose warfarin is better than placebo or antiplatelet agents in preventing stroke (SOR: A, meta-analysis of RCTs). Newer agents for anticoagulation (apixaban, dabigatran, rivaroxaban) are as effective as adjusted-dose warfarin for preventing stroke and may be associated with a decreased risk of intracranial bleeding (SOR: B, individual RCTs).
