When should you consider implanted nerve stimulators for lower back pain? Evidence-Based Answer: Spinal cord stimulation (SCS) appears effective in more than half of patients with chronic back and leg pain, with or without failed back surgery syndrome. Patients who have had symptoms the longest tend to obtain less relief (SOR: B, systematic review of RCTs and non-RCTs). For patients with leg-predominant radicular neuropathic pain, SCS in addition to conventional medical management (CMM) is more effective than CMM alone (SOR: B, RCT). Patients with back-predominant pain may experience relief with high-frequency SCS (SOR: B, open-label trial).
