What is the most effective management of an acute fracture of the base of the fifth metatarsal? Evidence-Based Answer: For an avulsion fracture of the fifth metatarsal, a soft dressing or walking boot is better than casting in reducing pain and time to baseline function. Surgical treatment is preferred for a fracture of the proximal fifth metatarsal involving the junction of the diaphysis and metaphysis, because of the high incidence of nonunion, delayed union, or refracture with nonsurgical treatment (SOR: B, RCT and prospective comparative study).
