Evidence-based answer: In addition to hydrochlorothiazide (HCTZ), other thiazide, thiazide-like, and loop diuretics increase the risk of incident gout. Potassium-sparing diuretics such as spironolactone do not appear to be associated with an increased risk of gout (SOR: B, systematic review of RCT, cohort trials, and consistent cohort and case-control trials).
