Does 17 alpha-hydroxyprogesterone (17P) prevent delivery before 37 weeks in women with singleton gestations and risk factorsfor preterm birth? Evidence-Based Answer: Weekly intramuscular (IM) injection of 250 mg 17P is effective for preventing delivery before 37 weeks in women with prior spontaneous preterm birth (SPTB), but not in women with short cervical length (SOR: A, meta-analysis of RCTs). In women with prior SPTB and short cervical length, 17P does not prevent delivery before 37 weeks with or without cervical cerclage (SOR: B, cohort subgroup analysis of RCT). There appears to be no difference in starting 17P between 16 and 20 weeks or between 21 and 27 weeks in women with prior SPTB (SOR: B, retrospective cohort analysis).
