Is chronic NSAID use safe in patients who have baseline CVD? Evidence-based answer: In patients with baseline cardiovascular disease (CVD), chronic use of NSAIDs is associated with CV mortality rates more than 2 times those of non-NSAID users. NSAID use is also associated with increased rates of CV events (SOR: B, retrospective cohort and post hoc analyses of RCTs). The American Heart Association (AHA) recognizes the CV risks of NSAIDs and recommends against NSAID use in patients with known CVD unless all other treatment options fail (SOR: C, expert opinion)
