Are intranasal steroids effective for symptomatic treatment of patients with sinus congestion? In acute sinusitis, intranasal steroids resolve or improve symptoms slightly better than placebo, with higher doses more effective than lower doses (SOR: A, meta-analysis of RCTs). In acute rhinosinusitis, fluticasone intranasal spray once or twice daily improves symptom scores slightly more than placebo (SOR: B, single RCT). Intranasal steroids are an option for viral rhinosinusitis and acute bacterial rhinosinusitis and recommended for chronic rhinosinusitis (SOR: B, evidence-based guideline).
