Right-heart catheterization is the gold standard for diagnosing pulmonary hypertension. Medical management should be based on functional status using supportive and specific therapies. (strength of recommendation [SOR] A, based on evidence-based guidelines.) If possible, patients should be referred to a center specializing in pulmonary hypertension.
