Patients who present with intolerable myalgia but a normal creatine kinase (CK) should discontinue statin therapy. When symptoms resolve, the physician can restart the same statin at a lower dose or attempt a different statin. (SOR C, based on expert opinion.) If low-density lipoprotein cholesterol (LDL-C) concentration is not at goal with reduced dosing or with the new statin, consider adding other active agents. (SOR C, based on expert opinion.)
