There is no significant difference in clinical cure or tolerability between short-course (7-14 days) and long-course (>14 days) antibiotics for acute uncomplicated pyelonephritis in adults. (SOR A, based on a meta-analysis.) A recent guideline from the American College of Obstetricians and Gynecologists (ACOG) recommends 14 days of antibiotic therapy, although there is limited evidence that 7-day therapy may be as effective in premenopausal women.
