What is the optimal interval for liver associated enzyme monitoring in patients taking statins for hypercholesterolemia? Evidence-Based Answer: There is no evidence that monitoring aspartate aminotransferase (AST) or alanine aminotransferase (ALT) in asymptomatic and otherwise healthy patients taking statins can prevent severe liver toxicity (SOR: B, based on retrospective cohort studies). Still, current FDA approved prescribing guidelines recommend checking AST and ALT prior to initiation of therapy, 3 months after initiation or dose increase, and periodically thereafter for asymptomatic individuals (SOR: C, based on consensus guidelines).
