Should we screen for coronary artery disease in patients with type 2 diabetes mellitus?

Universal screening of patients with type 2 diabetes for coronary artery disease (CAD) using myocardial perfusion imaging (MPI) does not improve cardiovascular outcomes. (SOR: B, based on a single RCT.) Coronary calcium scoring in this group does correlate with all-cause mortality (SOR: B, based on a single cohort study), but it is unknown if clinical interventions would make any difference in outcomes. Current consensus guidelines recommend selective screening based on duration of disease and presence of additional cardiovascular risk factors. (SOR: C, based on expert opinion.)
