    Review Article : Neuromuscular causes of weakness in critically ill patients

    Multiple predisposing factors are responsible for the development of neuromuscular weakness in a critically ill patient. Thorough history and physical examination is paramount and should not be omitted despite that the majority of patients are unresponsive, confused or sedated. A comprehensive investigation protocol should be followed in all cases of weakness in ICU settings. The spectrum of neuromuscular diseases encountered in ICUs nowadays has rapidly evolved over the last decades. Early diagnosis and prompt treatment is necessary to reduce morbidity and mortality in critically ill patients and reduce the skyrocketing health care expenses.

    An information technology solution to provide a real-time alert to the nursing staff is necessary to assist in identifying patients who may have sepsis and avoid the devastating effects of its late recognition. The objective of this study is to evaluate the perception and adoption of sepsis clinical decision support.

    Case Reports : Topiramate, a concealed cause of severe metabolic acidosis

    Severe metabolic acidosis is common among critically ill patients, and topiramate is a rare cause that may fail recognition. We report a lady with acute encephalopathy who had severe non-anion gap metabolic acidosis that served as the clue leading to suspicion and diagnosis of topiramate toxicity and was confirmed by elevated blood topiramate levels. Additionally, we provide a review of literature on all reported cases of topiramate toxicity. Keywords: Topiramate, metabolic acidosis, migraine, altered mental status, carbonic anhydrase.
