Abstract
"We aim to facilitate the initiation of CHF care and transition processes for patients with CHF as early as possible in their hospitalization at MUHC. We plan to accomplish this through the development of a CHF early identification innovation, CHF nurse driven protocol, and a CHF Sub-Phase order set which will allow for earlier identification for the clinical care team to better prepare the patient for a successful transition home."--Aim statement.
