Abstract
"High-readmission rates of patients discharged to post-acute care (PAC) facilities are challenging hospitals across the nation as inefficient and ineffective transitions along the care continuum lead to poor patient satisfaction, sub-optimal clinical outcomes, and lost revenue. New alternative payment models compound the problem as Medicare incentives require hospitals to manage patient care for months after discharge. In response, many healthcare systems are building PAC provider networks to focus on quality, care coordination, and cost. Partnering with high-performing PAC facilities, driving performance improvement, and aligning strategic goals have helped healthcare systems implement best practices, create a positive patient experience, and maximize reimbursement."--Problem statement.
