Abstract
"As part of MU Health Care's pursuit of the Triple Aim (reduce costs, improve the health of populations, and improve the patient experience), MU Health Care implemented HealtheRegistries in March 2015. HealtheRegistries allows providers to manage chronic conditions and wellness objectives across patient populations. It identifies gaps in care based on current evidence and displays them in the patient's chart. These tools help care team members to better manage their patients' health before, during, and after visits. During visits, a tool in the chart displays these gaps. Between visits, it identifies patients with critical follow-up items to proactively address them. In addition, HealtheRegistries tracks and displays the overall quality of care provided to patients. This enables quality improvement and measurement at the point of care. "--Health registries.
