Abstract

"To decrease the number of Emergency Department (ED) visits (and related costs) by 20 percent after the placement of a emergency acute care plan placed into the patient's chart to be used at each ED visit. This plan will be pre-approved by the patient, primary care giver and ED provider."--Aim statement.
