Abstract

"Our idea is to improve the management of patients with chronic health conditions by increasing patient engagement and patient activation. The chronic conditions specifically reviewed in this analysis include diabetes mellitus (DM), hypertension (HTN), arthritis, heart disease, chronic obstructive pulmonary disease, asthma, hyperlipidemia, chronic pain, and mental health. Patients with chronic conditions may suffer a higher rate of mortality, increased morbidity, and decreased quality of life. Needs assessment: Patients with chronic conditions can utilize large amounts of health care resources; which increase costs to the health care system."--Background.
